Community Health Needs Assessment

The Patient Protection and Affordable Care Act (PPACA) requires non-profit hospitals, such as Community Memorial Hospital to conduct a Community Health Needs Assessment (CHNA) for our community.

The CHNA process identifies key issues affecting health in our community, as well as health problems people in our community experience. This provides CMH with a foundation to develop a proactive plan to address those needs.

2025 Survey

The 2025 CHNA survey is open until March 31. It asks a series of multiple-choice questions and takes approximately 10 minutes to complete. Participation and responses are anonymous, and results are published on CMH’s website following the completion of the survey. Take the survey now
